Fort Bend Hospice & Palliative Care provides complete quality care to patients requiring comfort care. We provide care in the home or wherever a person chooses to reside. We offer an array of services and a team dedicated to providing care to all patients.
If you are diagnosed with a serious or life-threatening illness, the physical, emotional and psychological burden can be difficult to manage. Our Palliative Care specialists work with your treating physicians to ease the disease burden and provide you with the best possible quality of life. Our experts can assist in many ways:
We create a customized care plan tailored to meet the unique needs and preferences of each patient and their family. Our plan includes; Unlimited Practitioner visits, Medication Management, Labs drawn as needed, X-Rays, Respite Care with 24 Hour Licensed Nurse as well as Continuous Care.
Our team consists of compassionate and highly trained professionals who are dedicated to providing the best possible care. The team includes a physician, nurse, patient & family advocate, patient & family assistant and trained volunteers who will manage symptoms and pain and help guide treatment decisions.
We provide medical care you would normally receive at your doctor’s office, without having to go to the doctor’s office. Most of your medical services including laboratory and X-Ray, can be given at your home this enables patients to maintain their independence and experience life’s pleasures with their families.
The team includes a physician, nurse, patient & family advocate, patient & family assistant and trained volunteers. Will manage symptoms and pain and help guide treatment decisions. A nurse care manager or social worker will call or visit when you need their services.

The Palliative Care does not replace the attending physician but works as an additional layer of support with the patient’s physician to co-ordinate care. The Palliative Care team also works in close collaboration with hospital social workers and chaplains to help ensure optimum care.
Most people who use Hospice are over age 65 and are entitled to the Medicare Hospice Benefit, which covers virtually all hospice services, and requires little, if any, out-of-pocket costs. Thus, no financial burdens are incurred by the family, in sharp contrast to the huge financial expenses at the end of life, which are incurred when Hospice is not used. Hospice services are also provided by Medicaid and most insurance companies.
