• Convene family meetings to discuss care goals and to make difficult treatment choices.
• Coordinate care across healthcare services to maximize quality of life along the continuum of illness.

• Doctor visits patient monthly per patient request.
• A unique program delivering medical care to patients living with chronic conditions in the comfort of their homes.
• The team includes a physician, nurse, patient & family advocate, patient & family assistant and trained volunteers.
• Will manage symptoms and pain and help guide treatment decisions.
• Enables patients to maintain their independence and experience life’s pleasures with their families.
• Provides medical care you would normally receive at your doctor’s office, without having to go to the doctor’s office.
• A nurse care manager or social worker will call or visit when you need their services.
• Most of your medical services including laboratory and X-Ray, can be given at your home
• The ability to call us 24 hours a day, 7 days a week and speak with someone who knows your medical condition well.
